To help us provide the best possible care, please bring the following items to your first visit:
Referral letter from your GP or specialist
New Patient Form – print & complete [online here] or arrive 10–15 minutes early to fill it out at reception
List of current medications
Private health insurance details
Claim details (DVA, Workcover, or TAC), including claim number
Approval Letter from Insurer, Employer, TAC form consultation if relevant
Relevant imaging (X-rays, CT, MRI, Ultrasound, EMG) and radiology reports
If you don’t have a copy, please contact us in advance so we can arrange electronic access
Blood test results (if relevant)
Pathology, histology, or biopsy reports (if relevant)
Any letters or reports from other healthcare providers

We are situated at 114 David Street, Dandenong, opposite the Dandenong Hospital Emergency entrance. We have a small carpark at the rear of our building.
What to Expect at Your Consultation:
We listen carefully to your concerns and expectations.
Treatment options are clearly explained to help you make an informed decisions.
A surgery date can be discussed and scheduled if appropriate.
You will be provided with a quote for any out-of-pocket expenses.

Fees:
Initial consultation: $200
Cosmetic consultation: $250
Review appointments: $100
Payment methods: Cash, EFTPOS, Visa, Mastercard, AMEX
Medicare rebates will be processed on the spot following your appointment
Cancellations
If you need to reschedule or cancel, please give us at least 24 hours’ notice so we can offer the appointment to another patient.
